PRESTONSBURG, Ky. – Medicare patients in eastern Kentucky diagnosed with dementia will soon have new care options as ARH Memory Care will launch on July 1.
The program will initially focus on the Big Sandy region and will be based at ARH Medical and Specialty Associates, a department of Highlands ARH Regional Medical Center. It is part of the Centers for Medicare and Medicaid Services’ (CMS) new GUIDE (Guiding an Improved Dementia Experience) Model.
GUIDE, according to CMS, is a groundbreaking dementia care program designed to improve the quality of life for both people with dementia, as well as their unpaid caregivers. It aims to allow patients to safely remain in their homes rather than being placed in long-term care facilities.
“The impact of Alzheimer’s disease and dementia in rural Appalachia is significant and affects not only those diagnosed with the conditions, but also their families and caregivers,” said Rachel Jenkins, ARH Neuroscience Service Line Director. “Addressing the unique challenges faced by our communities requires a concerted effort to improve care navigation and provide support for caregivers.”
Following the GUIDE Model, ARH Memory Care will provide 24/7 access to a support line, as well as caregiver training, education and support services.
Under the guidance of program medical director Dr. Jason Madey, patients and their families will work with a dedicated care team and nurse navigator Mary Crum to develop a personalized plan of care. And as patients with dementia often have additional chronic conditions, ARH Memory Care will provide “care coordination,” which includes referrals for healthcare and social needs.
Because caregivers are often socially isolated and at a higher risk of developing their own health issues, they will receive educational support on how to navigate caring for both loved ones as well as themselves. Additionally, they will also have the opportunity to receive important respite services, allowing them to take time away when necessary.
Crum said she looks forward to the program kickoff but said the goal is to eventually grow the program throughout the ARH system.
“Our initial focus is Big Sandy, but the need is everywhere,” she said, referencing the approximate 6.7 million Americans living with Alzheimer’s or dementia. “We have an aging population in isolated communities, so this really is an essential service for both patients and those who care for them.
“We’re excited to be able to provide the support they need to help maintain their well-being.”
